Extremely Low Birth Weight Follow-Up
Health and Neurodevelopmental Status at 3 to 6 and 18 to 24 Months Corrected Age
Eligibility
- Infants born weighing less than or equal to 1000 grams or gestational ages less than or equal to 27 weeks, 6 days who survived until hospital discharge.

- 3 to 6 months corrected age and 18 to 24 months corrected age

- Health status
- Living situation
- Social determinants of health screening
- Support after discharge
- Medical and surgical re-hospitalizations
- Emergency Department visits
- Growth, vision, and hearing
- Developmental status, cerebral palsy, gross motor milestones
Data Submission
- Secure online data submission through the ELBW Follow-Up data entry tool on the VON website.
Reporting
VON reports and reporting tools help members improve counseling of parents, plan and advocate for resources to help families transition beyond the NICU, understand health care disparities, and identify areas for additional research and quality improvement.
Members can track performance over time to benchmark their practices and outcomes against previous years, subgroups of similar centers, and the network as a whole. Reports show health and neurodevelopmental status for infants at 3 to 6 months and 18 to 24 months corrected age.
Optional reporting groups can be created for targeted benchmarking across multiple centers.
Cost
- $500 annually for each NICU member
- $750 annually for members receiving group reporting
How to Join
- Please check the Member Mapto see if your center is a VON member
- Verify that your center has been a member of the Very Low Birth Weight Databaseor Expanded Database for more than three months

- Centers that are not VON members need to join the Very Low Birth Weight Databaseor Expanded Database before joining the Extremely Low Birth Weight Follow-Up Database
